How do I get a copy of an incident report?
You may send a written request to the: Clay County Sheriff`s Office, Records Section, PO BOX 548, Green Cove Springs, FL 32043 or call the Records Section at (904) 213-6090,(904) 529-6090 or (352) 473-4807. When requesting a report, you must specify on your request if you wish to pick up the report at the station or have it mailed to you. Incident reports cost 15 cents per page.
